The first issue you have to fix is that poisoning weapons/ammunition requires an action. So you want to have an ample amount of weapons or ammunition you just poison before a fight and swap through. The second issue is that you only have a limited supply of poison (10 at level 1, 45 at level 10 if using all reagents).
